Band 6 Acute Liaison Nurse (ALN) Intensive Health Outreach Team (IHOT)

Overview
The role involves close collaborative working with the acute trust including awareness raising, identifying and meeting training needs and the provision of specialist nursing advice. You can expect specialist clinical training, development opportunities, regular supervision and professional leadership and a warm welcome. The post holder will be responsible for providing guidance to acute hospital staff on meeting the needs of people with learning disabilities in Shropshire and Telford and Wrekin, advising on reasonable adjustments to receive appropriate treatment.Responsibilities
To be responsible for community or hospital admissions, including liaison with other agencies, service users and carers.To evaluate the outcome of nursing interventions to ensure that care plan objectives have been met and that health gains have been achieved.To co-ordinate care with the MDT and hospital staff at points of attending appointments, admission and discharge.Any interventions must be within the post holder''s scope of clinical practice ensuring that training is maintained and skills are in line with current guidance.To provide advice, practical support and guidance with regards to people who may lack capacity to make decisions about their health.To develop risk assessments if needed and appropriate risk management plans.To undertake or facilitate assessments of patients before admission.To promote awareness of health issues and the development of health action plans with service users and their carers.To support the acute hospital services in the assessment, planning, implementation and evaluation of care plans for service users with highly complex needs.To facilitate and promote people with a learning disabilities understanding of their own health needs.Promote the active participation of service users, their families and carers in the health care process.Contribute in identifying barriers for people with a learning disability accessing health care provisions and promote initiatives to overcome these barriers.To have an understanding of other professional roles, both in the community and within the acute trust, in order to work collaboratively meeting the needs of the person with a learning disability.Provide leadership to hospital staff in preparing people with a learning disability for hospital treatment e.g. consultation, explanations of treatment, using different forms of communication.Advocate on behalf of service users and carers to ensure that client''s care needs are met and that all concerns are escalated to an appropriate level. Promote the needs and rights of people with learning disabilities including the right to and consent to treatment relating to their mental capacity.Candidate profile
We are looking to appoint an experienced, enthusiastic and motivated RNLD who is committed to supporting the needs of people with learning disabilities to take on the role of Acute Liaison Nurse for people with learning disabilities at Princess Royal Hospital Telford. The role requires someone who is able to demonstrate excellent inter-personal communication skills and a determined commitment to person-centered, multi-disciplinary working. This post offers the opportunity to work in a highly focused and supportive way with those people with learning disabilities who have complex physical health needs when admitted to or discharged from hospital.The post holder will be responsible for providing guidance to acute hospital staff on meeting the needs of people with learning disabilities in Princess Royal and also, on occasions, the Royal Shrewsbury Hospital, advising on reasonable adjustments to receive appropriate care and treatment.Experience of working with people with learning disabilities in the community is desirable. You will also preferably have experience of working with people who have complex needs, for example those associated with mental and physical ill health, autism and behaviours that challenge.About the Trust
