This At Home Service - Integrated Care Social Worker role will be for an initial 3-6 months with potential for extension subject to internal circumstance and your performance.At Home Service - Integrated Care Social Worker Responsibilities include:To represent my team in triaging and collaborating with other professionals like the OT and doctors to triage cases appropriately.To represent my team in MDT meetings to update the whole triaging team about the current state of cases that has been assessed and accepted as requiring social care intervention.To complete Care Act assessment and review of needs to identify the needs of our service-user and provide the right interventions. Example, packages of care.To make referrals to other Agency for support. Like Age UK, Assistive technology support,To complete mental capacity assessment under the MCA 2005Experience preferred / Minimum Requirements:Post qualifying experience of working as a Hospital or Initial Response Social WorkerSWE Registration1 year post qualifying experience within UK Social Care ..... full job details .....
