Locum Reablement Assessor Community-Based Service, 37 Hours Per Week, Flexible Shift Pattern We are currently seeking an experienced and motivated Locum Reablement Assessor to join a busy community-based service supporting individuals to regain independence and achieve positive outcomes following hospital discharge or within the community. This is an excellent opportunity for someone with a strong background in adult social care, reablement, or short-term support pathways who is confident working autonomously within a fast-paced multidisciplinary environment. The Role As a Reablement Assessor, you will deliver strengths-based assessments and coordinate short-term, outcome-focused interventions designed to maximise independence, recovery, and wellbeing. You will work closely with individuals, families, therapy teams, health professionals, and community partners to identify needs, develop reablement plans, manage risk, and ensure effective support is in place. Key Responsibilities Undertake strengths based assessments and reviews to identify needs and desired outcomes Develop and coordinate person-centred reablement plans promoting independence and recovery Provide advice, guidance, and signposting to community and statutory services Manage risk and safeguarding concerns using Making Safeguarding Personal principles Work collaboratively with therapy teams, health partners, and internal colleagues Support hospital discharge processes, equipment provision, and referral ..... full job details .....
