You will carry out assessments, develop personalised support plans, and help clients reconnect with accommodation, support networks and services to achieve positive long-term outcomes. Key Responsibilities Conduct comprehensive assessments and develop tailored action plans with clients. Provide ongoing case management, support and advocacy. Support clients to access accommodation, benefits, healthcare and other essential services. Coordinate reconnection arrangements, including travel, accommodation and support packages. Work closely with local authorities, outreach teams and partner agencies. Assess risks and develop appropriate risk management plans. Maintain accurate case records and update electronic databases.
