This is an exciting opportunity for an experienced social work professional who is passionate about delivering outstanding outcomes for adults and supporting the development of practice excellence across teams. As an Advanced Practitioner, you will play a key leadership role, providing professional guidance, mentoring colleagues, and ensuring high-quality, person-centred interventions for adults with complex needs. The Role You will: Manage a complex caseload involving adults with a range of social care needs. Provide expert advice, consultation and support to social workers and team members. Lead on complex assessments, safeguarding enquiries, Mental Capacity Act assessments and best interest decisions. Promote strengths-based practice and person-centred approaches. Support the delivery of high-quality care and support planning. Act as a practice leader, contributing to service development and quality assurance. Mentor, coach and support less experienced practitioners to enhance professional practice. Work collaboratively with health partners, providers, carers and community organisations.
