We work with people with learning disabilities and/or autism who have high acuity and complex needs. The team is composed of two hubs: the Preparing for Adulthood (PfA) Hub and the Community Inclusion Hub. Together, we work closely with a dedicated team of Enabling and Transitioning Officers, Independence and Wellbeing Officers, Technical Assistants, an Autism Engagement Officer, and a Co-production Lead. Our main focus is to assess individuals'' care and support needs and co-produce packages of care and support that promote independence, wellbeing, and inclusion. Working Pattern/ Approach Monday to Friday, based at County Hall, Cross Street, Beverley, HU17 9BA, with some travel required.
