Adults Social Worker- Single Point of Access

Our Client, a Welsh council is seeking an Adult''s Social Worker to join their Single Point of Access Team. Fantastic payrate of £34.31 per hour! Responsibilities of this role: Act as the first point of contact for Adult Social Care, screening and triaging referrals from professionals, service users, carers, and partner agencies. Undertake strengths-based assessments to identify needs, risks, outcomes, and eligibility under the Care Act 2014. Identify and respond to safeguarding concerns, completing initial enquiries and making appropriate referrals in line with safeguarding procedures. Provide information, advice, and signposting to community resources, preventative services, and support networks to promote independence and wellbeing. Carry out risk assessments and implement immediate safeguarding or support measures where urgent intervention is required. Work collaboratively with health services, housing, police, voluntary organisations, and internal teams to coordinate effective support and timely interventions. Maintain accurate case records, assessments, and decision-making documentation in accordance with legislative requirements and organisational policies. Requirements of this role: Must be a Qualified Social Worker, registered with Social Work England.
