Adults Social Worker- Older People

Our client, a Welsh Council, is seeking an Adults Social Worker to join their Older People Team. Fantastic payrate of £33.42 per hour! Responsibilities of this role: Undertake strengths-based assessments for older adults, identifying care and support needs, risks, and desired outcomes in accordance with the Care Act 2014. Manage a caseload of older people with a range of physical health, cognitive impairment, frailty, and age-related support needs. Complete Mental Capacity Assessments and Best Interest decisions where appropriate, ensuring compliance with the Mental Capacity Act 2005. Safeguard vulnerable adults by undertaking safeguarding enquiries, assessing risk, and coordinating protective interventions where abuse or neglect is identified. Work collaboratively with health professionals, families, carers, care providers, and community services to develop and review effective support plans. Arrange, monitor, and review care packages and commissioned services to ensure needs are met and independence is promoted. Maintain accurate case records, reports, and assessments, ensuring all interventions comply with statutory duties, legislation, and local authority policies.
