Conduct assessments (psychosocial, risk, care needs) to establish diagnoses and identify required support. Develop and monitor individualised care plans, in collaboration with clients and multidisciplinary teams. Undertake crisis intervention and risk management, safeguarding vulnerable individuals using frameworks like the Mental Health Act, Mental Capacity Act, or Care Act.
