Adults Social Worker - Learning Disabilities and project work
Our client Cambridgeshire County Council is looking for an Adults Social Worker to join their Learning Disabilities and project work service.We are seeking an experienced and motivated Locum Social Worker who will work closely with the Senior Social Worker and the Contracts and Brokerage Team to deliver high-quality, person-centred support within a specialist service.This role sits within a county-wide specialist team working with adults with Learning Disabilities and Autism (LDandA). A key focus of the role is reviewing complex and high-cost care packages to ensure they are delivering positive outcomes, remain proportionate to need, and represent good value for public resources.Key ResponsibilitiesComplete Care Act 2014 assessments, including complex and high-cost care packagesUndertake timely reviews following changes in individuals'' needs or circumstancesDeliver a minimum of two completed and recorded assessments per weekCarry out face-to-face assessments and reviewsActively involve advocates, carers, and familiesAnalyse and evaluate care and support arrangementsWork closely with brokerage and contracts colleaguesEnsure care packages maximise independence and wellbeingMaintain accurate and defensible recordsEssential RequirementsQualified Social Worker with Social Work England registrationExperience with Learning Disabilities/AutismKnowledge of Care Act 2014 and Mental Capacity ActExperience reviewing complex care packagesAccess to own vehicle is essentialBenefits of ..... full job details .....
