Adults Social Worker - Learning Disabilities and Autism
Our client Telford and Wrekin Council is looking for a Qualified Social Worker to join their Adults with Learning Disabilities and Autism team.
About the Role
As a Qualified Social Worker, you will manage a caseload of adults, delivering strengths-based assessments, support planning, safeguarding interventions, and reviews to promote independence, wellbeing, and positive outcomes.
Key Responsibilities
- Manage a professional caseload, completing assessments, support plans, reviews, and risk assessments.
- Apply strengths-based and asset-based approaches to support individuals and families.
- Promote safeguarding and undertake investigations where required.
- Work collaboratively with statutory, voluntary, and community partners to achieve agreed outcomes.
- Attend and contribute to peer reviews, reflective practice sessions, and team discussions.
- Maintain accurate case records, reports, and documentation.
- Provide professional advice and guidance to colleagues and support less experienced staff and students.
- Undertake duty responsibilities as part of the team rota.
- Promote equality, dignity, choice, and person-centred practice.
Additional Requirements / Expectations
- Qualified Social Worker registered with Social Work England.
- Knowledge of Adult Social Care legislation and best practice, including the Care Act 2014 and Mental Capacity Act 2005.
- Ability to make evidence-based decisions and effectively manage risk.
- Commitment to continuous professional development and reflective practice.
- Strong partnership-working and communication skills.
