The role involves working closely with health and social care professionals to ensure patients can leave hospital safely and with appropriate support.Key responsibilitiesComplete assessments of adults'' social care and support needs.Develop and review care and support plans.Support safe and timely hospital discharge.Assess patients'' ability to manage safely at home and identify appropriate support.Arrange or coordinate care packages, residential or nursing placements and community support where appropriate.Work with patients, families, carers and other professionals to identify solutions.Undertake safeguarding assessments and respond to concerns about abuse or neglect.Promote independence, choice, dignity and person-centred care.Participate in multidisciplinary team meetings and discharge planning.Liaise with hospitals, local authorities, care providers, community teams and voluntary organisations.Maintain accurate and confidential records in accordance with professional and organisational requirements.Complete assessments and make appropriate professional decisions in line with relevant legislation and local ..... full job details .....
