The roleIn this Social Work role you will complete Care Act assessments and participate in Multi-Disciplinary meetings to consider eligibility for Continuing Health Care for individuals who have been discharged from hospital with Discharge Support funding. These individuals will typically be in a bed based setting. You will work to defined timescales and promote interventions based upon the client''s strengths and focus on what is important to them, offering a truly holistic strength based approach. You will have the ability to engage with service users, their carers and system partners to undertake these assessments virtually and to commission appropriate care to promote independence and meet identified needs. The post on offer is a permanent, 37 hour contract working Monday to Friday.
