This is to work in a Social Care Connect Team The Social Care Connect Team is the front door for Adult Social Care at North Lincolnshire Council. As a Senior Social Worker within SCCT, you will: Manage and oversee the transfer of complex and occasionally contentious cases, working effectively with a range of stakeholders who may have differing priorities. Allocate short-term interventions and planned assessment work promptly and appropriately to ensure effective service delivery. Review worker caseloads and workflow trays to ensure work progresses in a timely manner and aligns with the Adult Social Care Model of Working. Facilitate regular Model of Working and transfer meetings to identify cases suitable for progression to long-term teams. Authorise Mental Capacity Act (MCA) assessments, V3 assessments, and BID workflows through the management system to support effective decision-making and service delivery.
