Adults Senior Practitioner - Community team
Our client Knowsley Borough Council is looking for an Adults Senior Practitioner to join their Community team.
Job Purpose
To provide high-quality, strengths-based social work practice to adults, their families and carers within a community setting. The Senior Practitioner will manage complex and high-risk cases, undertake assessments and reviews, develop person-centred care and support plans, and promote independence, wellbeing, choice and control.
The postholder will also provide professional support, advice and practice guidance to social workers and other members of the multidisciplinary team, contributing to the continuous improvement of social work practice and service delivery.
Key Responsibilities
Social Work Practice
- Undertake comprehensive strengths-based assessments of adults with care and support needs in accordance with relevant legislation, policies and procedures.
- Develop, implement and review person-centred care and support plans.
- Manage a complex caseload, including situations involving significant risk, safeguarding concerns, mental capacity, self-neglect, hospital discharge and multiple or complex needs.
- Promote independence, prevention, reablement and community-based solutions wherever appropriate.
- Work collaboratively with adults, families, carers and professionals to identify strengths, outcomes and available resources.
- Apply professional judgement when making decisions regarding eligibility, risk, support and appropriate interventions.
- Ensure adults'' wishes, views and desired outcomes remain central to decision-making.
- Undertake safeguarding enquiries and contribute to safeguarding planning and risk management where required.
- Apply the Mental Capacity Act and associated principles appropriately, including undertaking or contributing to capacity assessments and best-interest decision-making.
- Make appropriate use of advocacy and ensure people''s rights and participation are promoted.
Senior Practitioner Responsibilities
- Provide professional advice, consultation and practice guidance to social workers and other colleagues.
- Support less experienced practitioners with complex assessments, risk management and decision-making.
- Provide case supervision, mentoring and/or practice development in line with the requirements of the service.
- Promote high standards of recording, assessment, care planning and review.
- Assist with the allocation, prioritisation and oversight of complex work where required.
- Contribute to team discussions, professional forums, audits and practice improvement activity.
- Support the Team Manager with the development and implementation of service priorities.
- Act as a source of professional expertise within the multidisciplinary team.
