The Ageing Well Service provides assessment and review functions for older aged adults and older adults with mental ill health. This is an exciting opportunity for a dynamic Social Worker to work in and around Southend City with the aim of offering the appropriate support in the right place and right time. The main aim of the team is to use a strengths-based approach to enable people to live safely in the community, maximising their abilities to care for themselves and to support carers to continue in their caring role as long as they are able to. The team works creatively with people and their carers to empower them and identify individual and community-based assets. To understand an individual''s strengths, assets and wishes to maximise their potential for independence, recognising the importance of assuming that the individual is best placed to know their own outcomes, goals and wellbeing.
