Adults Advanced Practitioner - Integrated neighbourhood team
Our client Blackburn with Darwen council is looking for an Adults Advanced Practitioner to join their Integrated neighbourhood team.
Experienced social worker required for integrated neighbourhood team.
Requirements of role To support team manager with day to day running of the team and deputise as needed alongside undertaking statutory assessments.
Leadership & Supervision: Support, guide, and mentor social workers, assessment officers, and review staff.
Assessments & Reviews: Oversee and conduct complex Care Act assessments, manage support planning, and evaluate the suitability of commissioned care packages.
Multi-Agency Collaboration: Represent adult social care alongside health professionals, managing funding pathways and integrated care needs
Safeguarding- Undertaking s42 enquiries and leading on strategy discussion, provide advice and guidance to staff.
You will have a positive attitude to transformation, innovation and growing new ways of working. You will work closely with senior colleagues within the department and the wider council and will have the credibility to be able to provide healthy challenge, reflection and to be able to influence the development of our services.
