Location: Telford & Wrekin
Working Arrangement: Hybrid – Office based on Mondays, with home and community-based working

We are seeking an experienced Adult Practitioner to join a busy and supportive Adult Social Care team in Telford & Wrekin.

This role offers the opportunity to make a meaningful difference by supporting adults to maintain their independence, improve their wellbeing, and access the support they need through strengths-based and person-centred practice.

• Complete Care Act assessments, reviews and reassessments
• Undertake Carers Assessments and Mental Capacity Assessments
• Contribute to Best Interest decision-making processes
• Develop outcome-focused support plans that promote independence
• Identify and manage risk, supporting positive risk-taking approaches
• Support safeguarding activity and contribute to service improvement
• Work collaboratively with families, carers, health professionals and partner agencies
• Promote access to community resources, assistive technology and personal budgets

• Experience working within Adult Social Care
• Strong assessment, review and care planning skills
• Knowledge of the Care Act, Mental Capacity Act and safeguarding procedures
• Enhanced DBS with Adults and Children's Barred List
• Full UK Driving Licence and access to a vehicle
