We''re looking for an Adult Nurse Case Manager to join a leading national complex care provider, based from their Fazakerley branch and supporting packages across Merseyside and West Lancashire. You''ll take ownership of a caseload, combining your clinical expertise with leadership, assessment and care management. What you''ll be doing:Managing and reviewing complex care packagesCompleting clinical assessments and care plansVisiting clients in their own homesProviding clinical leadership to care teamsSupporting and mentoring nurses and support workersWorking with families and wider healthcare professionalsEnsuring high standards of clinical governance and careWhat you''ll need:Registered General Nurse with active NMC registrationAdult complex care experienceRecent tracheostomy and ventilator care experienceExperience supervising or supporting care staffStrong clinical assessment skillsFull UK driving licencePrevious Case Management experience isn''t essential.
