We offer high quality services that support both recovery and wellbeing, and we actively work with people so that they can be in control of their lives.    We now have an opportunity for an Admissions Avoidance Coordinator to join us to work closely with people who have experienced mental health difficulties and are being supported through BSMHFT Children’s and Young People Community Hubs and Crisis services.    The main aim of the service is to offer an individualised service with a focus on the prevention of hospital admission and provide support for service users.    You will work closely with the other Discharge Coordinators providing support cover when required and you’ll work closely with BSMHFT CYP staff to identify individuals who are at risk of escalation because of hardships including social and environmental factors.
