This is a locum opportunity offering flexible working and the chance to make a real difference in a supportive, multi-disciplinary team. Key Responsibilities: Provide person-centred social care interventions to adults with mental health needs Carry out assessments, care planning, reviews, and safeguarding enquiries in line with Care Act duties Work collaboratively within a multi-disciplinary team that includes nurses, psychologists, psychiatrists, and occupational therapists Liaise with families, carers, and partner agencies to ensure coordinated care Maintain accurate records and documentation in accordance with professional standards and Trust policies Participate in team meetings, reflective practice sessions, and ongoing service development What Youll Receive: Ongoing professional support from the Senior Social Worker and Team Leader Access to reflective practice sessions and regular supervision Support from a dedicated Social Care Lead ..... full job details .....
