18+ Leaving Care Personal Advisor

We are currently working with a client in Birmingham to recruit a Leaving Care Personal Advisor to support young people as they transition from care to independence. This is a rewarding role focused on empowering care leavers to achieve positive outcomes in education, employment, housing and wellbeing.Role Overview:As a Leaving Care Personal Advisor, you will work closely with young people aged 18-25 who are leaving or have left care. You will provide practical, emotional and professional support to help them develop independence skills and navigate adult life.You will act as a key point of contact, coordinating services and advocating on behalf of young people to ensure their needs are met.Key DutiesBuild positive and trusting relationships with care leaversProvide one-to-one support and guidanceAssist with housing applications and accommodation issuesSupport with education, training and employment opportunitiesHelp young people develop independent living skillsKey ResponsibilitiesDevelop, review and monitor Pathway PlansWork in partnership with social workers and other agenciesAdvocate on behalf of young people to access servicesPromote wellbeing, resilience and positive outcomesMaintain accurate records and case notesEnsure work is in line with relevant legislation and policiesContract Length: 3-6 months with a view to extend to an ongoing contract.
