Adults Social Worker - Older People and Hospital Discharge

Adults Social Worker required in Conwy County Borough Council. Social Workers will be expected to deliver a high quality social work service to individuals who are deemed vulnerable or needing support. You will need to work with the service user, carers and families to provide support and to develop resilience and Independence ensuring that the service users, and families are at the centre of decision making about their lives and the outcomes they want to achieve.

* Coordinate and manage hospital discharge processes for adult patients.
* Conduct comprehensive assessments for patients in hospital, at home, and in care homes.
* Develop and implement individualised care plans.
* Collaborate with health care professionals, families, and community services to ensure seamless transitions.

Requirements:
* Registration with Social Work England.
* Proven experience in hospital discharge processes.
