About the Role: As a Social Worker within our Reablement Team, you will play a crucial role in supporting the REaCH service to meet service demands. You will achieve this by completing reviews, Care Act assessments, and Mental Capacity assessments. Critically, you will progress individuals with eligible Care Act needs onto further long-term commissioned services/packages of care. This rewarding role requires strong assessment skills, excellent communication abilities, and a commitment to person-centred practice. Key Responsibilities: Work within a reablement team/service to monitor service users'' progress and assess any ongoing needs. Hold a short-term caseload, monitoring, prioritizing, and reviewing cases effectively. Conduct assessments under the Care Act 2014, including formal mental capacity assessments when required. Respond to statutory and non-statutory safeguarding concerns. Provide information, advice, and signposting to other agencies/services.
